Jetstar Airways A takeoff incident in was caused by a forgotten screwdriver tip, left in the Airbus jet engine, Australian investigators reveal in a report. Jetstar Airways Airbus A jet flight crew had to abort the takeoff due to engine power loss and low speed back in October There were passengers and six crew members on board. Shortly after the push-back, the first officer taxied the aircraft for takeoff on the dedicated runway 01L. Once the flight crew received approval for a takeoff, they lined up the plane and set a particular engine thrust level, which was counted based on the VH-VFF weight, flap settings, and the runway length available at the time. In the meantime, the plane instantly deviated to the right of the runway centreline. To stop the diverged aircraft, the first officer pressed the left-hand rudder pedal in full but the move did not help, so the captain selected reverse thrust and the jet decreased the speed to 30 knots.

The Jabiru, with a pilot and passenger on board, was tracking to the south-west of Ballina en route to Evans Head when their vertical separation reduced to approximately feet m as their flight paths intersected, with no observed lateral separation. Measures to further reduce risk might include the increased use of controlled airspace, the increased use of ADS-B aircraft surveillance data, both by air traffic services and in-aircraft, and the identification of any increasing risk through the improved monitoring of the quantity and complexity of aircraft movements in Ballina airspace. The investigation notes that a Civil Aviation Safety Authority review of the airspace surrounding Ballina determined that a higher airspace classification, such as introducing controlled airspace in the vicinity and an air traffic control service at Ballina was not appropriate. However, the ATSB found that that review and periodic risk assessments of the airspace did not consider the risks associated with aircraft transiting through the airspace without taking off or landing at Ballina, as occurred in the investigation occurrence. CASA has advised the ATSB that its most recent review of Ballina airspace is expected to take into account data for transiting aircraft that had either filed a flight plan or been detected by secondary surveillance radar. In addition, CASA has advised it has also developed an airspace risk modelling system that will have an enhanced capability to consider transiting aircraft. Separately, the investigation notes that at the time of the occurrence, pilots of aircraft operating within 10 nm of Ballina were required to make positional calls on the common traffic advisory frequency CTAF. In addition, in August , Airservices Australia introduced a surveillance flight information service SFIS , where an air traffic controller based in the Brisbane Air Traffic Services Centre provides traffic information but not traffic separation to aircraft operating within the Ballina 15 nm CTAF broadcast area. Jetstar passengers stranded on plane at Alice Springs Airport for more than six hours due to medical emergency. Passengers on an international flight from Thailand have spent more than six hours stranded on the plane at Alice Springs Airport, after it was forced to land due to a passenger suffering a medical emergency. Jetstar flight JQ30 was travelling from Bangkok to Melbourne when it was forced to land at the domestic airport at about 7. While the aircraft was on the tarmac, an electrical fault was detected which a Jetstar spokesperson said required a replacement part to be sourced from Sydney. The passengers were stuck on board the plane for almost seven hours while Jetstar attempted to arrange permission for them to disembark. With the plane suffering air conditioning issues and the on-screen entertainment switched off, passenger Paul Tarrant said the mood onboard was "getting a little bit crazy" as they waited to leave the aircraft. Passengers were finally given the option to disembark "into a specially partitioned section of the airport" at about 2. Typically, interiors feature seating for eight with a full-sized lavatory, or a slightly denser arrangement for ten. The JetStar has a fairly typical business jet design layout, with a swept wing and a cruciform tail. This meant the Jabiru was not providing the Jetstar flight crew with the necessary information to avoid the potential collision. TikTok faces potential ban in US US lawmakers unanimously approved legislation that could ban the popular platform. The investigation found that the pilot of the Jabiru had no idea the Jetstar flight was so close to him, or that the two aircraft were converging, until he flew just above the Airbus.
