I think this is absolutely hilarious the fact that's how Oda killed her off. So, naturally I don't think she's dead. Personally, I used to live in a two-story house and played on the stairs back when I was in middle school and I've fallen down those stairs on more than one occasion. The longest I was hurt was a few hours. Kuina dying from that is unthinkable unless she somehow managed to land on her neck or committed suicide [so was actively trying to kill herself]. Neither seem to be the case from what I understood from the story though. Unless Zoro didn't actually cheer Kuina up, but I feel like Oda would have made that more clear if that was the case. I would assume she is dead, just the way isn't necessarily how they said she fell down the stairs. She was extremely sad when she knew the fact that her breasts are hindering her path of becoming the best swordsman in the world, and even sadder when she overheard her father stating it outright. However, this is usually how Oda leaves himself with a plot option that he can twist in maybe another decade. He deliberately left out the evidence of her death so in the future if he decides to get her back, he can. And everyone would be like "wow, Oda's skills at foreshadowing is just godly".
