Knuckleduster is a towering, heavily muscled man. He has short dark hair and visibly thick eyebrows, and also possesses heavy stubble and a prominent scar running diagonally down his left cheek. His costume consists of a simple wrap-around black mask tied over the top of his head, a tight-fitting dark shirt, jeans, boots, and a long hunter green trench coat. On his hands, he wears a pair of dark gloves topped with a pair of brass knuckles, or knuckledusters, from which he derives his name. After his initial encounter with Number 6 , his right leg is busted to the point of requiring a cane to walk, and also appears to have his left eye completely scarred over. As O'Clock he wore a full-body red suit and a mask that covered his eyes and half of his face.

He is the main antagonist in the titular Vs. Hero Killer Arc. Chizome was imprisoned in Tartarus , but had escaped alongside most of the other inmates after All For One 's attack on the prison. The torso was sleeveless, so he wrapped his arms in yellow bandages from just below his shoulders to his wrists. Chizome also wore long black wristbands and a watch on his left arm, and his knees were protected with metal pads; black boots with steel armor and spikes completing the costume. He always donned his signature blood-red scarf, matching headband, and the tattered cloth he wore around his face as a mask. After being freed from Tartarus by All For One and returning to society alone, Stain had donned a white jacket with oversized sleeves. His forearms were wrapped with bandages, and he wore red gloves, dark navy blue pants, red metal pads around his knees, and black boots with white pads. He had two belts, one black worn diagonally on his torso where he held an old katana, and the other red worn around his waist holding what appears to be a small machete. His face was covered with a tattered cloth, leaving a part of his hair visible, which was tied into a ponytail. As Stendhal, his hair was even longer and less messy than it is currently and was tied into a neat ponytail.
