Nightmare Balloon Boy is a secret animatronic, an antagonist in Five Nights at Freddy's 4, and one of the seven nightmare animatronics eleven if the Halloween Edition animatronics are counted in the game. While he is one of the few new nightmare animatronics introduced in the Halloween Edition, nightmare bb, nightmare bb Nightmare Mangle andNightmarionne, he also makes an appearance in the Standard Edition. He is the nightmarish incarnation of BB. He is featured in his nightmare bb known as "Fun with Balloon Boy" and functions exactly the same as Plushtrap.

He is a nightmarish version of the original BB. He can be unlocked by beating Foxy Fighters with an A rank. Nightmare BB's appearance seems identical to his regular counterpart from the core series, but smaller and "cuter", with shorter legs and larger eyes with slightly small pupils. Nightmare BB is an extremely powerful character to use and one of the two characters that is needed for the Geist Lair. Nightmare BB has Mega Bite that can do huge damage to an enemy and others nearby. This can last between one to two minutes. Nightmare BB's best attack however, is Balloons 2.

This includes large, pointed teeth inside of his large mouth similar to Nightmare Chica , dark-blue eyelids and teeth gums, long claw-like fingers, and more sinister-looking eyes with red irises similar to Nightmare Chica's Cupcake as well as narrow gray veins. In fact, his mouth goes all the way around his head. He has a red triangular nose and circular cheeks instead of orange ones. He also has a red and purple striped hat and shirt, instead of his usual red and blue stripes. The pinwheel from the top of his cap is also blue rather than orange with a yellow button up top. His two buttons are indigo instead of the original BB's black ones. His brown hair is slightly shorter than his original counterpart. His blue pants are lighter in color. If one looks carefully, one can see that Nightmare Balloon Boy seems to wear white socks. His shoes bear an exact resemblance to his original counterpart's, albeit they appear lighter in color. Interestingly, Nightmare Balloon Boy appears to be in a perfect and completely undamaged shape. As with the other nightmares, he has no qualms about torturing his victims with his bony, metal hands.

In addition, each balloon has a double chance for bigger damage, which is usually around damage per balloon.

The character, mechanic, or other occurrence this article represents appears in UCN, and thus does not appear in Freddy Fazbear's Pizzeria Simulator. He returns in the Ultimate Custom Night. BB becomes active on Night 2. With each of his movements, he can say, " Hello ," " Hi ," or simply laugh. Balloon Boy is the only animatronic in the game that will not kill the night guard, but rather he will make an annoyance of himself by disabling the Flashlight and Air Vent Lights all while a sound loop of him laughing will play until the player is killed or gets to 6 AM. If this occurs, the player is especially vulnerable to Foxy's attack, as he can no longer be stunned with the now disabled Flashlight. To prevent Balloon Boy from fully entering the office, the player must put the Freddy Mask on while BB is in the Left Air Vent, and wait for the sound cue that he has exited the vent. However, the player should be advised that the noise heard may possible be another animatronic entering or exiting the Office or vents.
