The long-term survival of ACC patients is substantially superior to that of pancreatic adenocarcinoma patients. As there are no significant patient series about ACCs, our understanding of this illness is mainly based on case reports and limited patient series. Surgical resection is the treatment of choice for patients with the disease restricted to one organ; however, with recent breakthroughs in precision medicine, medicines targeting the one-of-a-kind molecular profile of ACC are on the horizon. There are no standard treatment protocols available for people in which a total surgical resection to cure the condition is not possible. As a result of shared genetic alterations, ACCs are chemosensitive to agents with activity against pancreatic adenocarcinomas and colorectal carcinomas. The role of neoadjuvant or adjuvant chemoradiotherapy has not been established. This article aims to do a comprehensive literature study and present the most recent information on acinar cell cancer. Core Tip: Acinar cell carcinoma ACC of the pancreas is a rare, malignant neoplasm that accounts for a small percentage of all pancreatic neoplasms. Our understanding of this disease remains unclear as there are no large series of patients with ACC.
Acinar cell carcinoma of the pancreas in the United States: prognostic factors and comparison to ductal adenocarcinoma.
Acinar cell carcinoma of the pancreas , also acinar cell carcinoma , is a rare malignant exocrine tumour of the pancreas. It typically has a guarded prognosis. The disease is more common in men than women and the average age at diagnosis is about ACC is associated with increased serum lipase and manifests in the classic presentation known as the Schmid triad subcutaneous fat necrosis , polyarthritis, eosinophilia. ACC are typically large, up to 10 cm, and soft compared to pancreatic adenocarcinoma, lacking its dense stroma.
Pancreatic islets, also called the islets of Langerhans, are regions of the pancreas that contain its hormone-producing endocrine cells. The pancreas serves two functions, endocrine and exocrine. The exocrine function of the pancreas is involved in digestion, and these associated structures are known as the pancreatic acini. The pancreatic acini are clusters of cells that produce digestive enzymes and secretions and make up the bulk of the pancreas. The endocrine function of the pancreas helps maintain blood glucose levels, and the structures involved are known as the pancreatic islets, or the islets of Langerhans. Pancreatic islets or islets of Langerhans : The islets of Langerhans are the regions of the pancreas that contain its endocrine hormone-producing cells.

The acinar cell of the exocrine pancreas has the greatest rate of protein synthesis of any mammalian organ. The acinar cell has a highly developed endoplasmic reticulum ER system combined with mechanisms to modify and transport newly synthesized proteins through the secretory pathway Figure 6 [ 2 , 26 ]. In addition to its functions in performing protein synthesis and processing, the ER is the major storage site for intracellular calcium, which, when released into the cytoplasm, is the mediator of regulated secretion of stored digestive enzymes into the pancreatic ductal system [ 27 ]. Electron micrograph of the pancreatic acinar cell. This electron micrograph shows the key cellular structures involved in synthesis, processing and storage of digestive enzymes.

Published online Oct Acinar cell carcinoma of the pancreas: clinical and computed tomography manifestations. Overexpression of c-src caused Golgi expansion whereas pharmacological inhibition reduced granule formation in cultured AR42J acinar cells and isolated acinar cells. Langenbecks Arch Surg. The cleavage is mediated by a specific ER protease signal peptidase and occurs within the translocon after a large portion of the nascent protein has entered the ER cisterna 3,6. Sci Rep. All cells in pancreatospheres retained an epithelial phenotype as confirmed by E-cadherin staining on day 4 Suppl. Prevalence and prognostic significance of acinar cell differentiation in pancreatic endocrine tumors. It is the co-expression of different markers that characterises a specific cell type and cellular state, e. The resulting mature secretory granules are stored until signals for exocytosis are received by the cell. Under normal physiological conditions, digestive enzymes are activated only once they have reached the duodenum. Molecularly guided treatments targeting oncogenic drivers and the DNA damage response and repair pathway require further prospective evaluation, based on these real-world findings. These results provide the first known signaling pathway for acute Golgi-mediated ZG formation in response to secretory stimulation. Utturkar, S.
The pancreas serves digestive and endocrine functions, and it is composed of two types of tissue: islets of Langerhans and acini. The pancreas is a glandular organ in the digestive system and endocrine system of vertebrates.
They can arise in any part of the pancreas. Furthermore, in 2D monolayer culture, epithelial-to-mesenchymal transition is known to occur, whereas E-cadherin expression was still present up to 14 days of 3D suspension culture with no upregulation of mesenchymal markers vimentin, N-cadherin, SNAI1 , ZEB1 and ZEB2 PanNETs are associated with a more favorable prognosis, making differentiation crucial. Ohike N, Morohoshi T. Reprints and permissions. Activation triggers Arf myristoylation and membrane tethering identical to Arf function in clathrin coat formation reviewed in Liu, J. Indeed, the expression of GP2 was reported to increase with acinar maturation 20 rendering this marker less adequate for acinar dedifferentiation studies but permit to investigate the cellular fate of acinar-derived cells. Golgi architecture in mammalian cells consists of a morphologically heterogeneous set of membrane-limited compartments with a characteristic stack-like appearance Figure 1. The microanatomy and exocrine functions of the model are characterized to confirm the normal acinus phenotypes.
